Job Summary:
The Housing & Shelter Case Manager provides comprehensive case management services to individuals and families engaged in Switchpoint’s emergency shelter and supportive housing programs. This role is responsible for guiding clients through the intake process, developing individualized service plans, coordinating access to resources, and promoting long-term housing stability and self-sufficiency.
The position is designed to serve both shelter and housing populations, with caseloads in the shelter program primarily focused on individuals and the immediate needs related to crisis stabilization. In contrast, the housing program includes both individuals and families, with an emphasis on long-term planning and the transition to permanent housing.

Job Responsibilities:
Intake Process:
- Completes an initial needs assessment and acts upon critical needs appropriately and immediately.
- Conducts comprehensive assessments of client needs, barriers, and strengths during the intake process.
- Orients clients to shelter and housing programs, including eligibility, requirements, services, and potential consequences.
- Assures that basic nutrition, hygiene, and safety needs are being met in both housing and shelter environments.
- Interviews and counsels prospective clients for the various Supportive Housing Programs Switchpoint offers.
Case Plans:
- Develops a comprehensive, client-driven case plan with both short-term and long-term SMART goals identified, aligned with overall program objectives.
- Monitors client progress through regularly scheduled sessions, home visits, and check-ins.
- Evaluates and adjusts case plans as needed, ensuring timely reassessments, and provides written warnings with consequences if satisfactory progress is not met.
- Provides individualized support in budgeting, employment planning, and referrals to relevant supportive services.
- Empowers clients to access appropriate resources to assist in achieving self-sufficiency and long-term housing stability.
- Assures that specific HUD and grant-related goals are consistently met and provides targeted case work to support the transition from homelessness to permanent housing.
- Ensures compliance with all housing and shelter program guidelines, including timelines and eligibility requirements.
Training Programs:
- Designs, coordinates, and implements training programs such as Life Skills, Tenancy 101, and Financial Literacy, aimed at improving life management and decision-making skills.
- Evaluates training effectiveness and monitors for behavioral and functional improvement.
- Establishes and coordinates day and evening programming; schedules sessions with outside service providers and volunteers.
- Participates in required trainings such as Crisis Intervention/Prevention, Bloodborne Pathogen training, and other role-specific development programs.
Recordkeeping and Reporting:
- Maintains accurate, up-to-date client files, including documentation of conversations, goal progress, warnings, incidents, and reassessments.
- Completes and enters all required documentation in a timely manner, including VISPDATs, SPDATs, case management logs, exit surveys, and incident reports.
- Collects and manages data to meet internal reporting, grant compliance, and statistical tracking requirements.
- Utilizes case management systems and Microsoft Office tools for timely and organized recordkeeping.
Property Management:
- Assures the safety and livability of both shelter and housing properties through routine facility inspections.
- Reports hazards immediately to the Executive Director and collaborates with resident managers and leadership to resolve safety or maintenance concerns.
- Facilitates the repair process in coordination with the Executive Director and Chief Financial Officer.
Teamwork and Collaboration:
- Works collaboratively with shelter staff, housing teams, and other agency departments to foster a client-centered, team-based service model.
- Participates in LHCC meetings, internal team meetings, and case reviews to support client success and program alignment.
- Creates and maintains strong relationships with landlords, housing authorities, welfare administrators, and area service providers to increase access to housing and support systems.
- Demonstrates effective, compassionate communication with clients, coworkers, and partners.
- Serves on committees and substitutes for other staff as needed.
Core Qualifications:
- Proficient computer skills including Microsoft Office and case management systems such as UHMIS
- Strong organizational and documentation skills
- Ability to work independently and manage a dynamic caseload
- Experience in Human Services, including working with a variety of vulnerable populations (e.g., individuals experiencing homelessness, mental illness, disabilities, substance use)
- Strong problem-solving, decision-making, and crisis response skills
- Excellent interpersonal skills with the ability to remain compassionate yet firm
- Familiarity with trauma-informed care, housing-first principles, and local resources
- Knowledge of TANF, RRH, and other housing-related programs
- Ability to remain flexible and calm under pressure
Education & Other Requirements:
- High school diploma or GED required
- Associate degree required; bachelor’s degree in Social Work, Human Services, or related field preferred
- At least 1 year of case management or relevant human services experience preferred
- Experience working with families or individuals experiencing homelessness or housing instability
- Bilingual (Spanish/English or other language) preferred
- Valid Utah Driver’s License and reliable transportation required

Compensation
Pay range between $18-$20/hour
