🔥 Looking for an ALL-STAR Home Health Community Liaison / Sales Manager for a 5-STAR Home Health Agency!

Location: Santa Clara, Alameda & San Mateo Counties | Full-Time

• Drive referral growth across hospitals, SNFs, ALFs, and physician offices.
• Represent a 5-Star Medicare-rated, Practitioner owned, clinically led agency at community events, health fairs, and professional networking functions.
• Partner with intake and clinical teams to ensure smooth referral-to-admission transitions.

Who We’re Looking For:

• 2+ years in business development, sales, or account management in home health, hospice, or senior living is REQUIRED. Unfortunately, you have to talk the talk and walk the walk .
• Outgoing, resilient, and goal-oriented — you thrive building relationships and growing territory presence.
• Organized, self-directed, and confident engaging diverse healthcare stakeholders.
• Valid driver’s license and willing to travel across Santa Clara, Alameda & San Mateo Counties .

Perks & Compensation:

• Base $80K–$90K + uncapped commission (OTE $155K+)
