Position: RN Case Manager Home Health
Service Area: Carson City

We are looking for an experienced
Registered Nurse (RN) Case Manager to join our growing team in

Your Role - Provide skilled, one-on-one nursing care in patients homes.
- Complete thorough OASIS assessments and design individualized care plans.
- Monitor progress, document accurately, and coordinate with the interdisciplinary team.
- Educate patients and families on care, medications, and self-management strategies.
- Collaborate with physicians, therapists, and aides for seamless care delivery.
- Participate in case conferences and quality improvement initiatives.
What Were Looking For - Active RN license in Nevada
- Minimum 4 years total nursing experience
- At least 2 years in a Medicare-certified home health agency
- Proficient in OASIS assessments and Medicare documentation
- Strong clinical judgment, communication, and organizational skills
- Reliable transportation with valid drivers license and current auto insurance
- Familiarity with EMR systems like Axxess (preferred)
