\n \n\n \nShift : Weekends: Every Friday, Saturday, Sunday and Monday, 10 hour days.
\n \n\n \nLocation: This position will do patient home visits in the Highlands & Hardee County Area.
\n \n\n \nThe community you’ll be caring for:
\n \n\n \n\nThe role you’ll contribute:
\n \n\n \nPerform the initial assessment of the patient and caregiver for the appropriateness of and acceptance for home health services; establishment and periodic review of the plan of care; implementation of medical treatment when ordered; referral follow-up, provision of, evaluation of and supervision of care; coordination of services given by other health care providers and documentation of all activities and findings. Completes documentation according to Organizational Policies and Regulatory guidelines, performs home visits as ordered. \n \n\n \n· Admit new patients according to Department procedure.
\n \n\n \n· Performs a comprehensive patient assessment, develop and implement an individual patient care plan based on ongoing interpretation of physiological, psychological, sociological deficits affecting the state of health of the patient.
\n \n\n \n· Evaluates patient progress toward care plan goals.
\n \n\n \n· Coordinates the activities of health team members to provide comprehensive care and continuity of services and conducts team meetings.
\n \n\n \n· Supervise and plan Home Health Aide activities appropriate to individual patient care requirements and in accordance with department policy.
\n \n\n \n· Supervise LPN activities appropriate to individual patient care requirements and in accordance with department policy.
\n \n\n \n· Demonstrates effective communication skills with all team member, patients and families.
\n \n\n \nThe expertise and experiences you’ll need to succeed:
\n \n\n \nMinimum qualifications :
\n \n\n \n· Graduate of an approved school of professional nursing
\n \n\n \n· Minimum of (1) year of clinical experience in Medical/Surgical nursing
\n \n\n \n· Current Florida licensure as a Registered Nurse
\n \n\n \n· Current BCLS
\n \n\n \n· Valid Driver’s License
\n \n\n \nPreferred qualifications:
\n \n\n \n· Two (2) years of Home Health nursing experience
\n \n\n \n· Bachelor’s Degree in Nursing
\n \n\n \n· IV therapy skills desirable.
\n \n\n \n· Venipuncture skills desirable.
