Now Hiring: Hybrid Nurse Practitioner (Full-Time) – Sacramento, CA

We’re seeking a dedicated Nurse Practitioner to join one of the region’s leading kidney care organizations in partnership with a value-based health network. This is a unique opportunity to step into a full-time hybrid role that combines telehealth and clinic-based patient care in a supportive environment that values collaboration, innovation, and quality outcomes.

Position Highlights:

• Full-time, Monday–Friday schedule (no nights, weekends, or call)
• Hybrid model with a mix of in-person and telehealth visits
• Self-scheduled patient appointments for greater flexibility

Responsibilities:

• Provide holistic care to patients with chronic and complex conditions, including CKD, COPD, diabetes, congestive heart failure, and dialysis patients
• Conduct in-person and telehealth visits, developing individualized treatment plans
• Collaborate closely with a multidisciplinary team of physicians, specialists, and allied health professionals
• Deliver patient-centered care with a focus on quality outcomes in a fast-paced setting

Qualifications:

• Active Nurse Practitioner license
• Experience in Primary Care or Internal Medicine preferred
• Value-based care experience strongly preferred
• Strong clinical judgment, communication, and organizational skills
• Compassion for serving patients with chronic conditions
