JOB TITLE: Licensed Practical Nurse (LPN)
REPORTS TO: RN Supervisor and Director of ADS
SIGN-UP BONUS $1500 to be paid $750 first 6 months/ $750 after completing next 6 months.
Full-Time and Part-Time positions.
JOB SUMMARY:
To ensure the efficient day-to-day operation of the nursing program for clients utilizing Agency Directed Services. This position supports the Certified Manager, RN Supervisor, and other staff. The LPN is expected to uphold and promote the vision, mission, and values of The Independent Living Center (TILC). The LPN shall provide passion for serving seniors, people with disabilities. Creative thinker with a proactive approach to problem-solving.Friendly and professional demeanor with excellent interpersonal skills.
PRIMARY DUTIES AND RESPONSIBILITIES:
Direct Services:
Deliver high-quality customer service in alignment with TILCs values and standards.
Apply the nursing process to assess, evaluate, plan, and implement client care. Responsibilities include:
Conducting new admissions
Completing general health assessments
Performing on-site 10% supervisory visits
Evaluating the adequacy of the Plan of Care to meet client needs
Provide nursing services as authorized by the Missouri Department of Health and Senior Services (DHSS), which may include:
Filling a one-week supply of insulin syringes
Setting up oral medications
Monitoring skin conditions
Providing nail care
Evaluate attendants ability to effectively follow the Plan of Care.
Communicate any changes in client condition or necessary adjustments to the Plan of Care to the Certified Manager.
Collaborate with other healthcare providers, including the clients physician, as directed by the client.
Comply with the code of ethics and maintain confidentiality.
Perform other related duties as assigned.
QUALIFICATIONS:
Graduated of an accredited LPN program.
Current licensure with the Missouri State Board of Nursing.
Current CPR certification.
Valid Missouri drivers license and proof of current automobile insurance.
Ability to work independently, prioritize tasks effectively, and maintain strong organizational skills.
Excellent interpersonal and communication skills with the ability to interact respectfully with individuals from diverse backgrounds.
Ability to push, pull, lift, and carry up to 25 pounds.
