Responsibilities include advocacy, assessment, referrals, goal setting, transportation, care coordination with outside providers, assistance with employment, housing, financial applications, discharge planning, intake coordination, and collaboration with counselors and recovery specialists to promote treatment plan goals. Attends community-based meetings. Essential Functions Establish and maintain engagement with clients to meet treatment goals. Meet weekly with clients for care coordination and planning. Conduct assessments to identify needs, strengths, and barriers. Create care plans collaboratively with clients and the treatment team. Facilitate psycho-educational groups. Address clients’ health and social needs comprehensively. Follow up post-discharge. Facilitate referrals and linkages to services. Maintain professional boundaries. Collaborate with service providers. Provide transportation as needed. Complete documentation timely and according to standards. Coordinate discharge and aftercare planning in advance. Attend staff meetings and trainings. Assist in crisis management. Respond to provider calls within 24 hours. Adhere to all organizational and regulatory policies. Maintain respectful working relationships. Demonstrate knowledge of Dimock’s continuum of care and policies. Perform other duties as assigned. Competencies Prioritize responsibilities effectively. Knowledge/Skills Strong verbal and written communication. Professional representation of Dimock. Intermediate skills in Word, Excel, PowerPoint. Flexibility and multitasking abilities. Work Environment / Standards Operate in a clinical setting. Follow policies and regulations. Meet standards and pay attention to detail. Physical Demands Light; lifting up to 20 lbs., with frequent carrying. Position & Hours Work Tuesday to Saturday or Sunday to Thursday, with at least one weekend day; shifts start between 7 am and 11 am. Required Education Bachelor’s in human services, psychology, sociology, health services, or related field. At least one year in a substance use program. Preferred Experience Two years working with individuals with substance use, mental illness, or homelessness. Other This description is not exhaustive; duties may change.
